Job description

Clinical Care: Provide advanced clinical assessment, diagnosis and management of adults with complex, long-term conditions - including frail, elderly and housebound patients - in their homes, care homes and community healthcare settings. Deliver proactive clinical care, including falls risk assessment, polypharmacy review, and anticipatory care planning. Work closely with families, carers and social care teams to support older patients with complex needs. Leadership & Coordination: Lead and coordinate clinical care pathways with GPs, community nursing, care homes, therapy teams, social care and voluntary organisations. Support the development of proactive frailty identification and early intervention within the practice and PCN. Full details of all responsibilities are provided in the Job Description.
