Job Overview

The nursing team is a strong, passionate and dedicated team of RNLD’s who endeavour to improve the health outcomes for people with learning disabilities across Ealing. We are looking for a band 6 Learning Disabilities Nurse to join us for a fixed term period of 9 months to cover a secondment. If you are passionate about working with people with a learning disability and have experience of doing so, the CTPLD welcomes RMN’s and RGN’s to also apply.

The successful candidate will be responsible for implementing the nursing model to assess, plan, implement and evaluate care, ensuring people’s physical health needs are met; to support mainstream health services to meet the needs of people with learning disabilities and to support hospital admission and discharges.

You will work alongside the multi-disciplinary team including psychiatry, nursing, psychology, PBS, speech and language therapy, occupational therapy, physiotherapy to ensure a holistic approach is taken to manage health needs.

You will be provided with many exciting opportunities to further enhance your learning and development through nursing away days, annual trust nursing conferences, attending trust quality awards and training and development opportunities. Main Duties of the Job

  8. Support service users to manage their physical health needs
  9. Supporting GP’s to increase annual health check uptake and quality
  10. Supporting the dementia pathway
  11. Symptom management and end of life care planning
  12. Supporting weight management
  13. Promoting positive sexual health
  14. Promoting healthy lifestyle choices
  15. Partnership working with the MDT
  16. Attending weekly referral meetings
  17. Attending weekly case discussion meetings
  18. Management of clinical and operational management of a band 5 nurse
  19. Supporting student nurses in the role of practice supervisor and assessor
  20. Attending regular supervision
  21. Adhering to NMC requirements including revalidation

  • A minimum of 1-year post graduation experience in working as a learning disabilities nurse
  • Good understanding of the needs of people with learning disabilities
  • Experience of managing the physical health needs of people with learning disabilities
  • Basic understanding of mental health needs of people with learning disabilities
  • Knowledge and understanding of health care systems and structures
  • Experience of working in a community setting
  • LeDeR training
