Job summary

As a Community Learning Disabilities Nurse Specialist, working within our Community Team for People with Learning Disabilities, County Wide; Chippenham, Warminster, Salisbury, Marlborough, in Wiltshire.

This a Community Learning Disability Nursing role which will actively promote the health and wellbeing of people with learning disabilities through a range of specialist clinical and education interventions in line with all the relevant government policies and guidance e.g. Valuing People 2009, Transforming Care 2015.

Main duties of the job

This role supports the health and wellbeing of individuals with learning disabilities through specialist clinical care, education, and collaborative working.

Key Responsibilities

  • Provide expert advice and tailored interventions to individuals, families, and carers.

  • Manage a complex community nursing caseload, including those eligible for Continuing Health Care or Section 117 funding.

  • Work closely with multidisciplinary teams, care providers, and local authorities to ensure coordinated care.

  • Support service users with physical, emotional, and behavioral needs through thorough assessment, planning, and intervention.

  • Promote independence and informed decision-making through accessible health education.

  • Deliver therapeutic group sessions and respond promptly to emerging needs.

Training & Development

  • Mentor students and support staff, contributing to their growth and development.

  • Collaborate with other professionals to share knowledge and deliver joint training.

Additional Considerations

  • Community-based role requiring travel, often in rural areas (driving license essential).

  • Ability to manage varied, sometimes emotionally complex situations with empathy and professionalism.

  • Flexibility and resilience to work independently in dynamic environments.
