Job description

Do you have experience in supporting people? Do you know how to deliver a range of community activities? Yes, great! Then we may have the role for you.

A fantastic and highly rewarding opportunity has arisen for a Community Inclusion Facilitator to join our enthusiastic and passionate colleagues in Islington. This is a part-time (18 hours per week), permanent contract.

As a Community Inclusion Facilitator, you will be a valued member of a team, flexible and committed to supporting people. Your role involves developing and offering a range of responsive, direct, individual, and group activities to people with a learning disability accessing the Me Time Programme.

Key Responsibilities:
  1. Work in a way that enables the people being supported to make decisions and choices, and encourage trying new opportunities in line with the life each person chooses to lead.
  2. Ensure the plans for the people supported are person-centred, with full involvement in all aspects of planning and review.
  3. Be creative, dynamic, and adaptable to potential challenging scenarios related to the needs of the people supported.
  4. Ensure the safety and wellbeing of supported individuals, monitor their progress, and report on the impact of the programme on their lives.
  5. Complete all recording requirements promptly, ensuring high-quality reporting and case studies.
  6. Communicate with supported individuals with dignity and respect.
  7. Be part of a team, supporting colleagues to build community networks, sharing contacts and connections, and raising concerns through appropriate channels, especially regarding safeguarding.
  8. Be flexible and able to work from different locations and projects within the community.
