Sirona's Community Homeless Health Team supports individuals who are homeless, sofa surfing, and in temporary and emergency accommodation. We help 'bridge the gap' and promote re-engagement with health care.
Day to day, you will contribute to tackling health inequalities by working with the most vulnerable groups.
You will proactively engage with individuals who have identified health inequalities at various venues in local communities, helping them improve their lifestyle and overall health. You will be an experienced practitioner who, acting within your professional boundaries, will provide care for the patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care.
Working as part of the team and as an autonomous practitioner, you will be self-driven, demonstrating safe, evidence-based clinical decision-making to deliver compassionate expert care for patients who are homeless.
You will collaborate with other agencies, including health and social care professionals, voluntary staff, and police, to meet patient needs, support policy and procedures, and provide nurse leadership as required.
The role demands a unique practitioner with exceptional communication skills and an ability to adapt to challenging situations. You will need to quickly develop trust with this cohort of patients, encouraging them to access, utilize, and engage with services for their often complex needs.

Salary: £37,338 to £44,962 per annum, pro-rata.
