Job description

Sirona's Community Homeless Health Team

Supports individuals who are homeless, sofa surfing, or in temporary and emergency accommodation. We help 'bridge the gap' and promote re-engagement with healthcare.

Day to day, you will contribute to tackling health inequalities by working with the most vulnerable groups.

You will proactively engage with individuals who have identified health inequalities at various venues in local communities, helping them to improve their lifestyle and overall health.

You will be an experienced practitioner, acting within your professional boundaries, providing care from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care.

Working as part of the team and autonomously, you will demonstrate safe, evidence-based clinical decision-making to deliver compassionate, expert care for patients who are homeless.

You will collaborate with other agencies including health and social care professionals, voluntary staff, and police to meet patient needs, support policy and procedures, and provide nurse leadership as required.

The role requires a practitioner with exceptional communication skills and adaptability to challenging situations. You will need to quickly build trust with this cohort, encouraging engagement with services for their complex needs.
