Job summary

Kent Community Health NHS Foundation Trust is seeking a Healthcare Assistant to support urgent care services in the community, helping patients remain independent at home and avoid hospital admissions. You will work with nurses and therapists, deliver prescribed care, monitor health needs and support rehabilitation and activities of daily living.

The role includes baseline observations, glucose monitoring and wound care, with training provided.

Qualifications

• Requires Level 3 health qualification (NVQ/ QCF or equivalent).
• Recent healthcare experience desirable.
• Full UK/EU driving licence preferred.

Responsibilities

• Provide community-based care to help patients stay independent at home.
• Deliver elements of prescribed care under supervision of Nurses/ Therapists.
• Conduct baseline observations, glucose monitoring, wound care and falls prevention.
• Support patients with activities of daily living and rehabilitation activities.
