We are looking to recruit a band 3Healthcare Assistant to work 22.5 hours per week, within our busy NeighbourhoodTeam.
You need to be a motivated, flexible and resourceful. Youwill be supported by a highly skilled multi-disciplinary team in order todeliver high quality care, whilst supporting the patients with a variety ofconditions and needs, in their own homes.
You need to have excellent communication skills as you willwork closely with other professionals and outside agencies and family.
The post holder is required to hold afull driving licence and have use of a vehicle to work in a variety ofsettings.

Under the direction of qualified staff, engage with individualpatients to build a rapport and enable them to achieve maximum independencewith activities of daily living, which may include:
a. Washing and dressing, grooming, meal preparation, eating anddrinking and functional transfers i.e. on/off toilet, bed, chair.
b. Carrying out remedial activities to develop physical,perceptual emotional and cognitive function.
Following appropriate training and competency assessment, carryout specific tasks delegated by the qualified nurse or therapist, such as woundcare/dressings/insulin administration and medication administration in linewith the Trust policy
Fit and demonstrate the safe use of equipment, as directed by thequalified clinician
Work withother professionals within the team.
Liaise withexternal parties, such as social workers and GPs.

Provide a high standard of outcome orientated care and support,following a care / rehabilitation / recovery plan, under the supervision anddirection of qualified clinical staff for patients in their own homes.
Work in such a way to maintain a patients dignity at all timeswhile respecting their individuality.
Maintain accurate and contemporaneous records.
Where applicable, monitor and report the patients level offunctioning within therapeutic programme offered and report back progress.
Be actively involved in own appraisal / personal developmentprogramme.
Participate in clinical audit as appropriate.
Maintain equipment and report any faults or defects.
Meet thetravel requirements of the post.
