Our Bromsgrove Neighbourhood teamconsists of Community Nurses, Physiotherapists, Occupational Therapists,Healthcare Assistants and administrative staff; with GP and social carecolleagues working closely alongside. We pride ourselves in providingoutstanding care to housebound patients across Bromsgrove.

Main duties of the job

Working flexibly to provide the service deliveryneed across the local area (the service operates from 7:30am 6:30pm, Mondayto Sunday including bank holidays)

An enthusiastic, motivated and innovative thinkerto become part of a primary care facing community

The ability to work autonomously and work within amulti-disciplinary team

Experience of working in a fast paced and variedenvironment

While on duty you will either be working aloneor alongside colleagues to support delegated tasks in line with Trust standardsand core values. Your typical working day will include home visitsfor adults over 18 years needing a range of interventions. These include caringfor people reaching the end of their life, supporting personal care,participating in manual handling, recording observations and all types of woundcare- post operative wounds, leg ulcers, pressure ulcers and complex dressingregimes.

Job responsibilities

Key responsibilities will include:

Maintaining accurate and contemporaneousrecords in accordance with Trust policies/procedures; while maintainingconfidentiality at all times.

Ensuring a safe working environment byadhering to Trust policies and health and safety guidelines

Provide a high standard of outcome orientatedcare and support following a care/recovery plan under the supervision andguidance of qualified health care professionals

Maintaining effective communication systemswithin the integrated team, with other agencies, the voluntary sector, theclient and carer.

Person Specification
Skills & Abilities
  • A positive attitude and be able to act on feedback
  • Ability to communicate clearly and effectively in English in both verbal and written formats
  • Ability to cope with difficult/emotionally distressing/ challenging situations
  • Ability to work autonomously and as part of a team
Additional Criteria
  • Willing to undertake further relevant training programmes
  • Ability to meet the travel requirements of the post
  • You understand that we will not provide sponsorship to work for this role.
Qualifications
  • Level 3 qualification (e.g. NVQ 3, A-level) in Care or Level 2 qualification (e.g. NVQ2, GCSE Grade A-C) in Care and will be expected to undertake the relevant Level 3 qualification on appointment to post
  • Will be expected to meet the Care Certificate standards within 12 weeks of commencing employment with the Trust
  • I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
Knowledge
  • Basic knowledge of physical/mental health conditions
  • A level of understanding of rehabilitation
Experience
  • Experience of working with older adults in a health/social care setting
  • Community experience of older adults with physical and/or mental health problems
  • Experience of working and liaising with staff from different disciplines
