Job description

This role is part of the community nursing team visiting housebound patients in their own homes. Working within your competencies, you will undertake nursing interventions for patients on the caseload that have been allocated to you.

Working predominantly unsupervised, you will organise your workload to accommodate timed visits. You will follow a personalised care plan to deliver prescribed care, document your interventions within the clinical record, report back your findings, and escalate any concerns to the team.

You will participate in daily handover and undertake additional tasks required for the patients seen that day. Within the team, there are opportunities to engage as a champion or link worker, supporting the wider team.

Band 3 Senior Healthcare Workers will work closely with all service users, their families, friends, and carers, who contribute significantly to the patient experience. They are expected to be kind, responsive, professional, and informative, contributing to the quality of community services.

Key Responsibilities:
  • Keeping service users as safe as possible through sound clinical skills and effective risk assessments, escalating concerns to Registered Healthcare Professionals.
  • Ensuring the best clinical outcomes by using up-to-date skills and adhering to evidence-based policies within scope of assessed competence.
  • Providing a positive experience for service users by respecting, empowering, and working in partnership with them during care planning.

The service operates daily, including bank holidays and weekends, with shift patterns from 8 am to 10 pm, with some flexibility considered.

Rated ‘outstanding’ by the CQC, we are committed to high standards of patient care and excellent community services. We seek outstanding individuals who share our values: compassionate, aspirational, responsive, and excellent.

We value diversity and inclusion, believing they lead to better patient care and a happier workforce. We welcome applications from varied backgrounds and perspectives. We collect information on protected characteristics to promote equity, diversity, and inclusion, which is confidential and used solely for monitoring purposes.

As a flexible working organization, we support arrangements that suit both staff and service needs. Discuss flexible working options with us to find mutually beneficial arrangements.
