Join a leading community nursing team as a Band 3 Senior Healthcare Worker, providing essential care to housebound patients. You will work independently, ensuring safety and positive experiences through compassionate care. Flexibility in working hours and a commitment to high standards of patient care are key aspects of this role.
This role is part of the community nursing team visiting housebound patients in their own homes. Working within your competencies, you will undertake nursing interventions for patients on the caseload that have been allocated to you.
Working predominantly unsupervised, you will organise your workload to accommodate timed visits. You will follow a personalised care plan to deliver prescribed care, document your interventions within the clinical record, report back your findings, and escalate any concerns to the team.
You will participate in daily handover and undertake additional tasks required for the patients seen that day. Within the team, there are opportunities to engage as a champion or link worker, supporting the wider team.
Band 3 Senior Healthcare Workers will work closely with all service users, their families, friends, and carers, who contribute significantly to the patient experience. They are expected to be kind, responsive, professional, and informative, contributing to the quality of community services.
The service operates daily, including bank holidays and weekends, with shift patterns from 8 am to 10 pm, with some flexibility considered.
