Exciting Full-Time Opportunity for a Health Care Worker to Join the Heart Failure Specialist Team

We are looking for a dedicated Health Care Worker to join our Heart Failure Specialist Team. This position is ideal for someone with strong communication skills who is eager to expand their knowledge and experience in heart failure care, while working in a collaborative and supportive team environment.

In this role, you will work under the guidance and supervision of qualified professionals to support the needs of clients in Calderdale. You will also assist with daily administrative tasks to ensure the efficient operation of the Heart Failure Team.

The postholder will work as part of an integrated community-based service, under the supervision of the registered staff to manage their own caseload within both a community or clinic based setting. This may include contributing to the provision of education, advice and support to other healthcare professionals, agencies and members of the general public.

At all times treat patients, service users, families, visitors and colleagues with dignity and respect to deliver compassionate care, exercising discretion and behaving in a manner consistent with Calderdale and Huddersfield One Culture of Care.

To maintain confidentiality in all aspects of work and work within the Healthcare Support Worker Code of Conduct.

Work in a collaborative and co-operative manner with other members of the care team to meet the holistic needs of patients and carers.

To establish and maintain effective communication and relationships with the multi-disciplinary team, respecting the equality and diversity of each and every person he/she/they comes into contact within the course of his/her/their business.

Always uphold the privacy and dignity of patients.

Work in line with Trust policies and procedures and within scope of competence, undertaking mandatory training and essential skills training as identified for the role.

Contribute to the development of the clinical area, through contribution of new ideas to improve

patient care/experience and by escalating concerns appropriately.

We employ more than 6,500 staff who deliver compassionate care from our two main hospitals, Calderdale Royal Hospital and Huddersfield Royal Infirmary as well as in community sites, health centres and in patients homes. We also are incredibly proud to have almost 150 volunteers here at CHFT.

We provide a range of services including urgent and emergency care; medical; surgical; maternity; gynaecology; critical care; childrens and young peoples services; end of life care and outpatient and diagnostic imaging services.

We provide community health services, including sexual health services in Calderdale from Calderdale Royal and local health centres. These include Todmorden Health Centre and Broad Street Plaza.

We continue to modernise and invest in our health services to build on our strong reputation. Foundation trusts are public leaders in improving quality in health services. They are part of the NHS yet decisions about what they do and how they do it are driven by independent boards. Boards listen to their Council of Governors and respond to the needs of their members patients, staff and the local community.

Foundation trusts provide what the health service wants, yet are also free to invest quickly in the changes to the local community needs, in striving to be the best, and in putting their patients first.

  • Under the direction of the Registered Healthcare Professional, provide high quality personalised care in accordance with agreed local and national standards using a wide range of skills
  • Achieve and maintain core and specific competencies as per HCSW development pathway and agreed by manager.
  • Assist with the safe moving, handling and mobilisation of patients ensuring that care plans are followed to minimise risk.
  • Communicate effectively with kindness, warmth and compassion recognising when there are barriers to communication where support may be required.
  • Act as a patient and staff advocate, escalating concerns and challenging poor behaviour or practice appropriately.
  • Maintain accurate records of care within area of responsibility.
  • Observe and report any changes in a patients condition (including for example a deterioration in a patients clinical condition or any risks associated with health and safety) to the Nurse in Charge identifying and reporting patient needs not included in the care plan.
  • Involve patients and carers/relatives in delivery of care.
  • Provide reassurance and support to patients, carers/family.
  • Support the maintenance and cleanliness of the clinical environment by attending to general tidiness of the area including unpacking and storage of stock deliveries. Report any estates issues appropriately.
  • Support clinical teams to fulfil cleaning responsibilities as identified in the National Cleaning Standards framework.
  • Assist and contribute to the movement and treatment of patients to achieve physical comfort and minimise the adverse effects of pressure according to the plan of care.
  • Contribute to minimising the effects of disruptive abusive behaviour and the protection of individuals from abuse.
  • To answer the telephone and relay messages appropriately.
  • In the absence of administrative support, support administrative functions as directed by the nurse in charge .
  • Develop own skills and knowledge and contribute to the development of others.
  • Assist with the induction of new Health Care Support Workers
  • Recognise and respond appropriately to urgent and emergency situations.
  • Ensure the effective and efficient use of resources and contribute ideas to cost improvement programmes.
  • Report any incidents, concerns, complaints or accidents to the Nurse in Charge completing adverse incident forms as required.
  • Attend and participate in team meetings.
  • Support the psychological and emotional wellbeing of patients, carers and families.
