An exciting opportunity has arisen for an enthusiastic and motivated first-level registered nurse to join Fife’s Community Diabetes Specialist Nurse Service at 1.0 WTE.
Fife Community Diabetes Specialist Nurse Service operates Monday to Friday, 08:30 to 17:00, with a 60-minute lunch break. The service acts as an interface between Primary and Secondary Care for patients with complex diabetes, aiming to prevent unnecessary hospital admissions and support patient self-management.
We are committed to improving the patient journey through collaboration with other Health and Social Care and multi-agency teams. We work closely with Secondary Care Diabetes Service, including In-patient DSNs, to provide early supported discharge, and offer education sessions and support to professionals in GP practices, community nursing, care homes, and hospitals.
You will demonstrate a passion for diabetes care, excellent communication skills, and the ability to work autonomously and in a team. Evidence of recent CPD at degree level minimum is required, along with a recognized diabetes specialty module. Post-graduate study in diabetes is encouraged.
This role involves using various IT systems; proficiency in Microsoft Office, especially Excel, is essential. Reliable travel across Fife to multiple sites, including patient homes, is required.
If you meet these criteria, the service offers supervision, training, mentorship, and support to develop diabetes knowledge and skills, including insulin management and achieving TREND/WAND CSN competencies.
Applicants not meeting all criteria for Band 6 but demonstrating relevant experience and passion may be considered as Band 5 trainees under Annex 21, with progression to Band 6 upon achieving competencies and qualifications.
