Job summary

Wight Healthcare is seeking a Band 6 Community Diabetes Specialist Nurse to join our nurse-led team, working across community and primary care to support adults with diabetes.

You will provide specialist assessment, education and support, advise primary care on glucose-lowering therapies including insulin, and contribute to service improvement to reduce variation in care. We value autonomous practice and collaboration across settings.

Responsibilities

• Provide specialist assessment, education and support to adults living with diabetes across community and primary care.
• Offer specialist advice and guidance to primary care on diabetes management and therapy initiation/optimisation including insulin.
• Deliver education to patients and healthcare professionals and collaborate with colleagues across primary, community and secondary care.
• Contribute to service improvement and transformation to reduce variation in diabetes care across the locality.

Working across community and primary care settings, you will provide specialist assessment, education and support to adults living with diabetes, helping them to improve their health and reduce the risk of diabetes-related complications.

The role includes providing specialist advice and guidance to primary care, supporting the initiation and optimisation of glucose-lowering therapies including insulin, delivering patient and healthcare professional education, and working collaboratively with colleagues across primary, community and secondary care.

You'll also have the opportunity to contribute to service improvement and transformation, helping us to improve outcomes and reduce variation in diabetes care across the locality.
