A leading healthcare provider in Aylesbury is seeking a Diabetes Specialist Nurse to act as a clinical expert within their Specialty Service. This community-based role involves developing services, providing clinical support, and educating patients and healthcare professionals. The ideal candidate will have a Registered Nurse qualification and experience in a diabetes environment, along with strong communication skills and the ability to work with diverse client groups.
The Diabetes Specialist Nurse will act as the clinical expert and professional resource to the multi-professional team, patients, and carers within their Specialty Service. This role is primarily community-based.
The post holder will have a responsibility to help develop the specialist area of practice in partnership with other members of the specialty team, working collaboratively to develop services and improve the quality of care delivered.
The post holder will carry responsibility for a defined caseload and offer specialist clinical and service support and advice, including setting standards of care, contributing to specific care guidelines, and supporting the care of patients across the health economy. The post holder will also have an active role in the education of patients, carers, and healthcare professionals in their area of expertise.
The post holder will contribute to achieving the objectives of the clinical service group/specialty corporate objectives and work within the clinical governance framework. This will be achieved through the core dimensions of the role: clinical practice, leadership, acting as the patient advocate, and through audit and education.

Salary: £46,148 to £52,809 per annum, pro-rata.
