Job summary

Surrey Downs Health and Care is seeking a Band 3 Community Diabetes Health Care Assistant to join the Diabetes Team across Surrey Downs. You will support patient education and diabetes care, helping with clinics and diabetes-related health screening.

The role supports the Diabetes Team in delivering education to patients and clinicians, with induction and ongoing development opportunities. A driving licence is preferred for travel between sites.

Qualifications

• Must be able to demonstrate literacy and numeracy skills.
• Experience as a Health Care Assistant in a community or acute setting is desirable.
• DBS checks may be required and safeguarding responsibilities apply.

Responsibilities

• Support the Diabetes Specialist Team in providing patient education.
• Assist with health screening and checks related to diabetes management.
• Be a contact for patients receiving care from the community diabetes service.
• Contribute to improving patient experience and safety in diabetes care.
