Job summary

Discovery is seeking Community Connectors to join our Outreach team in Yeovil and surrounding areas. You’ll work in people’s communities, helping them develop skills, build confidence and pursue independence. The role is flexible with shifts and involves travel across Somerset.

No prior experience is required as full training is provided; you will complete a full induction and work towards the Care Certificate within the first three months, with support from Locality Managers.

Qualifications

• Adequate literacy and numeracy skills and basic computer skills.
• No direct experience required; full training provided.
• Induction starts on day one and you will work towards the Care Certificate within three months.

Responsibilities

• Encourage choice, control and personalised outcomes.
• Enable access to meaningful activities, hobbies and experiences.
• Build positive, trusting relationships that make a lasting difference.
