Job summary

Newport City Council is seeking a Community Connector to link Newport residents to community activities and support networks. The role is full time, within Prevention & Inclusion in social services, based at the Civic Centre, Newport.

It is a fixed term until 31 March 2027 with possible extension depending on grant funding. The post offers flexible working from home, office, and community venues; weekends/evenings not required.

Qualifications

  • Experience working within health and social care, wellbeing, education or a community development setting.
  • Commitment to prevention and early intervention.
  • Knowledge of Social Services Wellbeing Wales Act (2014).

Responsibilities

  • Provide information, advice and assistance to people aged 16+ who are vulnerable or isolated.
  • Engage with individuals 1:1 face-to-face and via telephone, email or post; undertake duty work.
  • Occasionally conduct home visits and attend community groups or events in Newport.

Are you hard working and looking for an exciting opportunity to work with Newport residents, linking them to community activities and support networks. Newport City Council is recruiting to a full time Community Connector post within Prevention & Inclusion in social services based at the Civic Centre. The post is fixed term until 31st March 2027 with a view to extending the role for a further year, depending on grant funding. The council offers flexible working from home, office and community venues during weekdays between 9am and 5pm. Weekends and evenings not required.

The purpose of the role is to provide information, advice and assistance to people aged 16+ who are vulnerable or isolated. These individuals may require engagement and support to improve independent living and tackle social isolation.

The successful candidate will work closely with health and social care services, voluntary sector partners and community groups. You will work on a 1:1 basis with individuals, providing face-to-face advice as well as on the telephone, by email or post. Duty work is also part of this role. Occasionally you will also be required to conduct home visits and attend community groups or public events in the Newport area. The team also supports unpaid carers.

Full training provided, however you will require experience of working within health and social care, wellbeing, education or a community development setting. You will be experienced in giving information and advice and engaging with vulnerable adults of all ages. You will be committed to the prevention and early intervention agenda and have knowledge of the Social Services Wellbeing Wales Act (2014).
