Job summary

Buckinghamshire Healthcare NHS Trust is seeking a Community Case Manager (Band 6) to join the Southern ACHT in Chalfont St Peter. The role covers home-based nursing interventions and caseload supervision within the community, with a 37.5 hour week and options for flexible working.

You will assess physical and psychosocial needs, coordinate visits, manage referrals, and develop clinical pathways in collaboration with a multidisciplinary team.

Qualifications

• Registered Allied Healthcare Professional or Nurse with current registration.
• Degree/Diploma in Health/Nursing Studies or related subject.
• Evidence of CPD and ongoing professional development.
• Post-graduate training in Elderly, Long Term Conditions, Respiratory, Cardiac, Dementia, Rehab, Trauma/Ortho and End of Life.

Responsibilities

• Provide home-based nursing interventions with caseload management and staff supervision.
• Coordinate visits and administer holistic care in the community.
• Proactively identify high-intensity users and manage referrals within the Adult Community Healthcare Team.
• Develop protocols and clinical pathways to support care delivery.
• Support mentoring and coaching to develop team skills.
