Ready4Care Support Ltd is an adult and children home care and supported living service provider serving various locations. We provide compassionate care and support for people who cannot wholly look after themselves in line with their care and support needs as defined by and usually assessed by the Local Authority under the Care Act 2014.
Our care and support could involve both regulated activity under the heading of personal care as defined by the Health and Social Care Act 2008 and unregulated activity in the form of help with general household activities such as cleaning and tidying, shopping or help with shopping, escorting to engagements and appointments, companionship and respite “sitting” services.

Tasks
- Assist service users with personal care tasks, such as washing, dressing, eating and toileting.
- Perform light housekeeping tasks, such as dusting, vacuuming and changing bed clothes.
- Assist in the safe lifting, transferring, repositioning and movement of service users.
- Accompany service users to medical appointments.
- Observe, monitor and record service users’ physical and emotional well-being, and promptly report any changes to senior staff.
- Encourage service users to participate in social and recreational activities.
- Motivate service users to be independent.
Main duties, responsibilities, and accountabilities
- Work with other members of the administrative team to ensure high-quality service provision.
- Maintain good communication with all relevant parties involved in the care of service users.
- Provide on-call service outside normal working hours and keep detailed logs of all on-call activity.
- Be prepared to go out on care calls when required, whether as part of a package or in an emergency.
- Contribute to the development of the Home Care monitoring program based on risk assessment.
- Ensure monitoring considers concerns, complaints, compliments, and feedback from service users, their representatives, social workers, and reviewing staff.
- Liaise with Social Workers about concerns relating to individual service users.
- Report any concerns regarding service provision, including possible breaches or abuse, to the Registered Managers immediately.
- Ensure all statutory training requirements are met.
- Assist line managers and staff in problem-solving, either individually or in groups.
- Stay updated on developments regarding disability issues and legislation through relevant journals, meetings, seminars, and exhibitions.
- Implement effective record-keeping practices.
- Undertake duties of a key worker, developing professional relationships with service users and their families.
- Participate in reviews and ensure recommendations are carried out.
- Assist with assessments and planning of individual service users’ needs and goals.
- Attend meetings as required and record information for reports.
- Establish and develop person-centred plans and health action plans in collaboration with relevant parties.
Requirements
Preferred skills and qualifications
- Experience caring for individuals with learning disabilities, Autism, dementia, or Alzheimer’s disease.
- Ability to communicate in more than one language.
- Professional certification such as Skills for Care, Care Certificate.
Additional requirements:
- Knowledge and experience working with the client groups mentioned above.
- Ability to support community activities.
- Experience in providing personal care, feeding, etc.
Person specification — essential criteria
- Honest, self-motivated, organized, caring, flexible, sensitive, and a good communicator.
- Able to follow care plans and instructions.
- Complete Care Certificate within 3-6 months.
- Enroll for QCF/RQF Level 2 after 1-2 years.
Person specification — desirable criteria
- Previous care work experience.
- Own transport.
- Knowledge of local neighborhoods and specific conditions like Autism, Learning Disabilities, Dementia, etc.
