Job summary

Moray Council is seeking a qualified Social Worker to join the Integrated Community Care Area Team. You will participate in holistic assessments of client needs and commission care packages, working across professional boundaries.

The role requires PVG clearance and SSSC registration, with travel across Moray by car as needed. The post involves liaising with health, social care, and partner agencies, maintaining records, and contributing to performance indicators.

Qualifications

• Experience in Health/Social Care settings.
• Demonstrable understanding of main issues facing Community Care.
• SSSC registered or eligible for registration.
• Ability to manage workload and meet deadlines.

Responsibilities

• Undertake Community Care assessments as required by Team Manager or referrals from professionals.
• Design and organise suitable packages of care with financial assessments and ensuring resources are available.
• Liaise with other services and agencies to meet needs in efficient ways.
• Maintain case records and contribute to performance indicators and statutory duties.
