Job summary

Moray Council seeks an experienced Social Work professional to participate in an Integrated Community Mental Health Team. You will undertake holistic assessments, commission packages of care and collaborate across professional boundaries to meet client needs.

The role involves financial assessments, resource planning with carers and agencies, and regular review of service provision. You will travel between Moray work locations as needed and contribute to team supervision and policy adherence.

Qualifications

Effective written and oral communication skills.
SSSC registration is required.
Experience in social care or health/social care settings is desirable.
Ability to work flexibly and manage workload.

Responsibilities

Undertake Community Care assessments as required by Team Manager or referrals.
Design and organise care packages with financial assessments and resources.
Liaise with local authority services and partner agencies to meet client needs.
Monitor and review service user needs and provision regularly.
Provide direct service and support to individuals, families and carers.
Collaborate with Health and Social Care Teams and participate in supervision and ERDP.
Maintain case records and provide information for performance indicators.
