Job summary

Responsibilities

• Undertake Community Care assessments as required by Team Manager.
• Design and organise care packages with financial assessments and available resources.
• Liaise with local authority services, partner agencies and carers to meet needs efficiently.
• Monitor and review service user needs and provision regularly.
• Maintain case records in electronic and manual formats; provide information for performance indicators.
