Job summary

Swindon Borough Council is recruiting a Social Care Practitioner for the Community Led Support Team (CLS). The role focuses on person-centred support, completing Care Act assessments, and coordinating support and reviews to improve wellbeing for adults in Swindon.

The role requires NVQ3 in Health or Social Care or equivalent, and the ability to travel across the Borough with access to a car for home visits. Hybrid working is in place, with in-office days required.

• You must be fluent in English and able to work in a fast-paced, supportive environment.
• Ability to travel throughout the Borough and access to a car for home visits is required.

Responsibilities

• Carry out Care Act assessments, support planning and reviews.
• Ensure person-centred care that promotes independence, wellbeing and outcomes.
• Conduct Mental Capacity Assessments and Best Interests decisions where required.
• Work collaboratively with colleagues, partner agencies and safeguarding teams.
• Support adults with care and support needs while meeting statutory responsibilities.
