Job summary

Woking & Sam Beare Hospice and Wellbeing Care is seeking a compassionate Community Staff Nurse to join the Hospice at Home team in North West Surrey. You will provide holistic assessment, care planning, symptom management and practical care for patients with palliative and end‑of‑life needs.

You will work with Clinical Nurse Specialists, GPs and District Nursing Teams to coordinate safe, person‑centred care, maintain accurate records, support families and contribute to bereavement support.

Qualifications

• Registered nurse with post-registration experience.
• Excellent communication skills and commitment to compassionate patient‑centred care.
• Experience in palliative care, oncology, community nursing or multi‑agency working is advantageous.

Are you a compassionate Registered Nurse looking to make a meaningful difference to patients and families at one of the most important times in their lives? We are looking for a Community Staff Nurse to join our Hospice at Home team, supporting the delivery of specialist palliative and end-of-life care across North West Surrey., In this role, you will provide holistic assessment, care planning, symptom management, emotional support and practical care for patients with specialist palliative and end-of-life care needs. You will work closely with Clinical Nurse Specialists, GPs, District Nursing Teams and other professionals to coordinate safe, responsive and person-centred care. You will also maintain accurate clinical records, support families and carers, contribute to bereavement support where appropriate, and act as a positive clinical role model for junior colleagues, students and new team members., Please refer to the job description attached to this advert for a full list of duties and responsibilities.
We are looking for a registered nurse with post-registration experience, excellent communication skills and a commitment to delivering compassionate, dignified and patient-centred care. You will be confident working both independently and as part of a multidisciplinary team, with the ability to manage a varied caseload and respond sensitively to the needs of patients, families and carers. Experience in palliative care, oncology, community nursing or multi-agency working would be an advantage, although we are equally interested in hearing from nurses who can demonstrate transferable skills, a willingness to learn and a passion for hospice care.,

• RN educated to Diploma or Degree level
• Palliative care qualification (or willingness to work complete)
• Mentorship qualification (or willingness to work complete)
• UK Registration
• Disclosure and Barring Service Check: The post requires a DBS check under the Rehabilitation of Offenders Act 1975 (Exceptions Order), Registered Nurse
• Post registration experience
• Experience of ongoing professional development
• Mentorship qualification or working towards, Experience of working in a MDT
• Knowledge of palliative care or relevant experience
• Knowledge of national, regional, and local policy in relation to quality standards, targets and improvements initiatives, Experience with multi agency working
• Experience in helping to develop clinical services, Excellent interpersonal and communication skills
