Job summary
Waythrough is seeking an Assertive Alcohol Outreach Worker to support individuals with long-standing alcohol use in the community. You will build trusting relationships, complete assessments, care planning and reviews, and work independently while collaborating with a multidisciplinary team.
A DBS check and travel across Northumberland are required. The role emphasizes proactive engagement, safeguarding, and accurate record-keeping as part of a broader support network for mental health,
Qualifications
- Experience within adult social care or a related setting.
- Ability to manage and prioritise a varied workload.
- Strong interpersonal skills and ability to build professional relationships.
- Resilience and ability to work in challenging environments.
- Full UK driving licence and access to a vehicle.
Responsibilities
- Provide outreach-based support to individuals in the community.
- Manage a small caseload with assessments, care planning and regular reviews.
- Deliver tailored support and work with a multidisciplinary team and external partners.
- Maintain accurate records and contribute to safeguarding processes.
- Collaborate with colleagues to ensure coordinated, effective support and safety.
