Coloplast is seeking an experienced Nurse with clinical expertise to develop and deliver community Stoma Care services, collaborating with Royal Liverpool University Hospital and supporting patients in the surrounding areas.

Qualifications

• Registered General Nurse with current registration.
• Desirable Nursing degree qualification.
• Expert clinician in Stoma management.

Responsibilities

• Deliver professional and ethical care to patients.
• Support patient pathways from hospital to home with home visits and community clinics.
• Plan, manage, and organise caseload and time efficiently.
• Communicate with patients, carers and referring Healthcare professionals.
• Undertake an Appliance Use Review (AUR) per guidelines.
• Maintain clinical records per requirements.
• Foster collaboration between acute and community health teams.
• Deliver educational support to healthcare professionals.
• Undertake service audit/evaluation activities.

We are looking for an experienced Nurse with relevant clinical expertise and a passion, drive, and commitment to develop and deliver community Stoma Care services. This role involves collaborating with our partners at Royal Liverpool University hospital and supporting patients in the surrounding areas.

The Role

• Deliver professional and ethical care to patients
• Support the patient pathway from hospital to home, providing home visits and community clinics
• Effectively and efficiently plan, manage, and organise caseload and time
• Communicate effectively and provide feedback to patients / carers and referring Healthcare professional
• Undertake an Appliance Use Review (AUR) where appropriate as per company and DoH Guidelines
• Maintain clinical records in accordance with Company, Legal and Professional requirements
• Instigate working practices to ensure collaborative working between different stakeholders e.g., acute and community health care teams
• Generate and maintain effective working relationships with health care professionals across the Region
• Deliver educational support to fellow health care professionals
• Undertake service audit/evaluation activities
