Job summary

East London NHS Foundation Trust is seeking a dedicated Co-occurring Mental Health & Substance Use Support Worker to join the CHIMES pilot in City & Hackney. The role focuses on proactive outreach, engagement, and practical support for individuals with complex needs.

You will work with Turning Point colleagues and CHIMES practitioners, manage a caseload, and receive training in motivational interviewing, harm reduction, and brief psychosocial interventions to support recovery and service

Qualifications

• Experience engaging with people with co-occurring mental health and substance use difficulties.
• Ability to work flexibly and independently.
• Willingness to undertake training in motivational interviewing, harm reduction and brief psychosocial interventions.

Responsibilities

• Manage an allocated engagement caseload under CHIMES supervision.
• Undertake outreach across inpatient and community settings, following up missed appointments.
• Build trusting relationships to encourage engagement with treatment.
• Provide practical and emotional support to overcome barriers to accessing services.
• Coordinate access to substance use treatment with Turning Point colleagues and conduct initial assessments after training.
• Maintain accurate records and contribute to care planning and safeguarding where needed.
