Job summary

East London NHS Foundation Trust is seeking a Community Support Worker to operate across the Community Mental Health Team (CMHT) and Memory Assessment Service (MAS). You will provide recovery-focused support to service users and carers, helping them access services, maintain wellbeing and work towards personal goals.

Working within a multidisciplinary team, you will assist with care coordination, observe physical health, facilitate group activities, signpost community resources and escalate

Qualifications

• Experience supporting adults in community mental health or similar settings.

Responsibilities

• Support care coordination and post-triage contacts.
• Assist with physical health monitoring and ECG/phlebotomy tasks after training.
• Signpost to community resources and contribute to MDT discussions.
