Job summary

Bradford District Care NHS Foundation Trust is seeking a Team Leader for the Craven Community Mental Health Team (CMHT). The role covers a large geographic area and requires leadership of a multidisciplinary team including nurses, social workers, OT's and Psychological Therapists.

You will balance clinical assessments and interventions with managerial responsibilities, developing partnerships with local services and ensuring high-quality community mental health care delivery.

Qualifications

  • Professionally recognised nurse, Social Services or AHP qualification.
  • Minimum of 3 years experience as a qualified professional in an acute mental health environment.
  • Experience in both health and social care.
  • At least 1 year management experience in community mental health service.

Responsibilities

  • Provide leadership, managerial and supervisory responsibility to a multidisciplinary team.
  • Oversee assessments and interventions within adult community mental health services.
  • Balance clinical work with management duties based on staffing.
  • Collaborate with GPs, other mental health services and partners.

We have an exciting opportunity for a Team Leader working at the Craven Community Mental Health Team (CMHT), covering a large geographical area. Our Community Mental Health Teams (CMHT) are always striving towards excellence in service delivery, ensuring quality is at the forefront of all we do and service users receive the right care at the right time. The successful applicant will be dynamic and forward thinking with the ability to work as part of a team and in partnership with local services across systems and partnerships. There will be an expectation that the successful applicant will uphold the Trust values at all times and act as a role model within the teams in providing quality care. The post holder will provide leadership, managerial and supervisory responsibility to a group of practitioners working as a multidisciplinary team including nurses, social workers, OT's and Psychological Therapists. providing assessments and interventions within adult community mental health services. The numbers of staff supervised will determine how the workload is managed between active clinical work and management responsibilities. The post holder will assist the clinical manager and work alongside the other team managers in continually developing the service and ensuring the delivery of an Community Mental Health Team Partnerships will include GPs, other mental health services, learning disability services, social care, physical health services, consultation to professionals in the primary care services and other adult mental health services including the voluntary sector.
