We are looking for enthusiastic and motivated Care Co-ordinators and can offer a range of exciting opportunities to work across community services and within our adult teams, developing our service as outlined by NHS England and Navigo.
Main duties of the job
- Supervise and coordinate the clinical care of a defined caseload.
- Assess, plan, implement, and evaluate care to provide appropriate interventions meeting individual needs.
- Assist in providing professional leadership, guidance, and advice to junior team members and other members of primary and specialist services.
- Maintain and enhance professional working relationships with referring agencies and colleagues from other disciplines and the voluntary sector, offering guidance on Mental Health issues.
- Be a member of the integrated Primary Care Network team CMHT, coordinating care across all disciplines and multi-agency boundaries.

Job responsibilities
- Clinical/Professional: Manage a caseload with patient-centered assessments and care planning; evaluate and participate in clinical audits; liaise with multidisciplinary teams; maintain confidentiality; adhere to relevant legislation and policies; work in various venues; participate in service duty system; act as Key Worker; and undertake related tasks.
- Education: Supervise and assess students; participate in educational development; serve as a professional role model.
- Managerial: Manage time effectively; support team leadership; supervise junior colleagues.
- Personal Responsibilities: Participate in performance reviews; adhere to health and safety policies; maintain a professional portfolio; perform other duties as delegated.
- General: Follow policies on health and safety, fire safety, equal opportunities, harassment, security, and confidentiality.
Person Specification
Knowledge
- Ability to write clear records and reports.
- Good communication, organizational, and administrative skills.
- Supervision and leadership skills.
- Knowledge of Community Care Act, Mental Health Act, and related policies.
- Experience representing the department in multi-disciplinary meetings.
- Previous supervisory experience.
- Additional experience or academic study related to mental health.
- Knowledge of organization procedures and early intervention in psychosis.
Qualifications
- Registered professional qualification (e.g., Nurse, OT, SW).
- Post-basic education/training in Mental Health.
- Knowledge of psychosocial interventions and evidence-based practice.
Additional Requirements
- Flexibility to meet patient needs.
- Appropriate dress.
- Valid UK driving license and access to a vehicle.
- Use of own vehicle.
Experience
- Experience in community mental health.
- At least 2 years at Band 5 in mental health.
- Experience in individual therapeutic environments and team settings.
