We are looking for enthusiastic and motivated Care Co-ordinators and can offer a range of exciting opportunities to work across community services and within our adult teams, developing our service as outlined by NHS England and Navigo.

Main duties of the job
  1. Supervise and coordinate the clinical care of a defined caseload.
  2. Assess, plan, implement, and evaluate care to provide appropriate interventions meeting individual needs.
  3. Assist in providing professional leadership, guidance, and advice to junior team members and other members of primary and specialist services.
  4. Maintain and enhance close professional working relationships with referring agencies and colleagues from other disciplines and the voluntary sector, offering professional guidance and advice related to Mental Health issues.
  5. Be a member of the integrated Primary Care Network team CMHT, coordinating care across all disciplines and multi-agency boundaries.

Job responsibilities
Clinical/Professional
  1. Manage a caseload, utilizing patient-centered assessment, planning, and care implementation.
  2. Evaluate care plans and outcomes, including participation in clinical audits.
  3. Collaborate with multidisciplinary team members to maintain professional relationships.
  4. Participate in regular clinical supervision per Navigo policy.
  5. Maintain confidential patient records.
  6. Work within Professional Practice Guidelines and Navigo policies.
  7. Comply with relevant legislation (Mental Health Act 1983, Mental Capacity Act 2005, NHS & Community Care Act 2014).
  8. Follow policies on medication management (for nurses).
  9. Engage in ongoing professional development.
  10. Coordinate with Primary Healthcare Teams and Care Programme Approach reviews.
  11. Work at various venues including clients' homes, hospitals, and GP surgeries.
  12. Participate in service duty system as required.
  13. Act as Key Worker.
  14. If qualified, act as an Approved Social Worker/Approved Mental Health Practitioner.
  15. Perform related tasks according to professional background.
  16. Must have a current UK driving license and access to a vehicle.
Education
  1. Supervise and assess students, supporting their learning objectives.
  2. Participate in educational activities to enhance mental health skills and knowledge.
  3. Act as a professional role model for learners and colleagues.
Managerial
  1. Manage own time effectively and support team leadership.
  2. Provide accurate information for organizational needs.
  3. Supervise junior colleagues through clinical supervision.
Personal Responsibilities
  1. Participate in performance and development reviews.
  2. Adhere to health and safety legislation and policies.
  3. Maintain professional registration portfolios.
  4. Perform other delegated duties within the scope of the role.
