The Heart Failure Specialist Nursing Team (HFSN) is pleased to offer an exciting opportunity for a newly created Band 3 Clinical Support Worker role supporting the service across South Warwickshire. The team is based at Acorn House, Warwick, providing clinics and home visits throughout the region.
The service operates Monday to Friday, 8:30 AM to 4:30 PM.
We are seeking a competent and well-organised individual with a particular interest in Heart Failure. The role involves managing patients diagnosed with heart failure and assisting in delivering personalized care plans emphasizing self-care.
The successful candidate will be motivated, enthusiastic, and demonstrate good interpersonal skills. As this is a community-based role, the ability to meet travel requirements is essential.
The Band 3 Clinical Support Worker will perform high-quality, compassionate delegated duties under the supervision of registered staff, working as part of a multidisciplinary team to deliver safe and effective patient care.
