Oxford University Hospitals NHS Foundation Trust in Oxford is seeking a dedicated healthcare assistant to undertake delegated clinical tasks and support patient care across wards.

You will prepare patients and families, assist with personal care, monitor wellbeing, document observations, and communicate effectively with the multidisciplinary team using SBARR; promote dignity and safety at all times.

Aufgaben

• Prepare patients, families/carers and the environment for clinical activities, supporting and reassuring them and preparing equipment as necessary.
• Assist patients with personal care needs, maintaining privacy, dignity and promoting independence.
• Monitor patients after clinical activities, ensuring safety, comfort and wellbeing.
• Undertake minor dressings and treatments as directed in care plans in consultation with a registered practitioner.
• Record observations and risk assessments, and report changes promptly to a registered practitioner.
• Assist in nutrition and hydration, including special diets and enteral feeding where required.
• Communicate effectively with patients and families, arranging information access and using SBARR as needed.
• Escalate concerns appropriately and participate in handovers to ensure continuity of care.
