Overview
Healthii People is recruiting on behalf of a supportive, training GP practice in Warminster, Wiltshire, seeking a Clinical Pharmacist to join their experienced team. This is a patient-facing role focused on medicines optimisation, repeat prescribing, and long-term condition management.

The postholder will be supported to enrol in the PCCE pathway and work toward an independent prescribing qualification if not already obtained.

Key Responsibilities

• Run clinics for long-term conditions and clinical medication reviews
• Support with care home and domiciliary medicines reviews
• Handle prescription queries and contribute to repeat prescribing systems
• Conduct medicines reconciliation post-discharge
• Help implement safety alerts, local guidelines, and audit recommendations
• Contribute to improving prescribing quality and safety across the practice

Requirements

• GPhC-registered pharmacist
• At least 2 years' experience
• Willing to undertake the PCCE pathway
• Team-oriented, with strong clinical and communication skills

Desirable

• Previous experience in general practice
• Independent prescriber (or training towards)
• PCCE pathway completed
