Job description

We are seeking a dedicated and proactive Clinical Pharmacist to join a team in Swindon. This is a fantastic opportunity to play a key role in delivering high-quality patient care within a dynamic and supportive healthcare environment. As a Clinical Pharmacist, you will work closely with GPs, nurses, and other healthcare professionals to provide expert medication management, improve patient outcomes, and support prescribing practices.

Key Responsibilities:
  • Provide expert clinical advice on the safe and effective use of medicines.
  • Conduct structured medication reviews to optimise patient treatment plans.
  • Support prescribing decisions to ensure cost-effective and evidence-based practices.
  • Offer guidance on long-term condition management, including diabetes, hypertension, and respiratory diseases.
About You:
  • A degree in Pharmacy (MPharm or equivalent).
  • Registration with the General Pharmaceutical Council (GPhC).
  • Experience in a primary care, hospital, or community pharmacy setting.
  • A postgraduate qualification in clinical pharmacy (desirable but not essential).
