Job summary

Addison Road Medical Practice in London is seeking a dedicated Clinical Pharmacist Prescriber to join our GP surgery. You will play a crucial role in ensuring safe and effective use of medications and work within a multidisciplinary team to enhance patient outcomes.

Ideal candidates will have an undergraduate pharmacy degree with GPhC registration, and experience in general practice and triage is desirable.

Qualifications

• Undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
• Independent prescribing qualification desirable
• Ability to apply effective clinical judgement in prioritisation and decision-making
• Previous general practice experience is a requirement
• Previous experience in triage is desirable

Responsibilities

• Prescribing, online, face-to-face and telephone triage, management of minor illness and LTC advice.
• Delivering safe, clinical decisions and expert care in a multi-disciplinary team.
• Work within the Triage Hub to assess and manage same-day patient requests.
• Provide administrative support in managing related demand.
• Conduct comprehensive structured medication reviews.
• Attend multidisciplinary case conferences and support long-term condition management.
• Manage risk stratification tools and repeat prescribing processes.
