Lakeside Healthcare is seeking a dedicated Clinical Pharmacist to join a multidisciplinary team across the Primary Care Network in Yaxley and surrounding practices. You will contribute to a forward-thinking organisation within the PCN framework and support service development and safe prescribing across sites.

The role involves structured medication reviews, patient-facing support, and leading quality improvement initiatives while ensuring continuity of care and adherence to NICE guidance.

Formación

• Registration with the General Pharmaceutical Council (GPhC).
• Evidence of CPD and ongoing professional development.
• Completion of CPPE pathway, or working towards completion.
• Independent prescriber status or progress toward it.
• Postgraduate diploma level or equivalent training/experience.

Responsabilidades

• Provide expertise in clinical medicines management and safe prescribing.
• Conduct structured medication reviews for polypharmacy patients.
• Offer patient-facing support to improve adherence and outcomes.
• Lead clinical audits and quality improvement initiatives.
• Develop and implement new services with medicinal components.
• Educate staff and monitor outcomes related to medications.
• Monitor local and national guidelines and formulary adherence.
