Job summary

Quinte Health is seeking a Pharmacist to join our hospital-based Pharmacy Department. The role involves clinical pharmacy practice across patient care floors, including potential involvement in Oncology and Antimicrobial Stewardship after additional training.

You will identify and resolve drug-related problems, provide drug information and therapeutic recommendations, and participate in rounds, discharge planning, and medication reconciliation with a focus on patient-centered care.

Qualifications

• Bachelor of Science in Pharmacy, Doctor of Pharmacy (PharmD) or undergraduate degree recognized by the PEBC.
• Registration as Part A with the Ontario College of Pharmacists.
• Hospital pharmacy residency or equivalent experience is an asset.

Responsibilities

• Provide clinical pharmacist services on assigned patient care floors, including Oncology and Antimicrobial Stewardship after training.
• Identify, resolve and prevent drug-related problems with a patient-centered approach.
• Provide drug information and therapeutic recommendations to healthcare professionals; intervene when needed.
• Engage in interprofessional rounds, discharge planning, and discharge management.
