Job summary
Peverell Park Surgery and University Medical Centre, a teaching practice across two sites, is recruiting a Clinical Pharmacist to its multidisciplinary team. The work centres on structured medication reviews, medicines optimisation and clinical reviews for patients with long-term conditions, with expert advice for clinicians.
The post also covers care home and frail patient reviews, and aims to free up GP time while improving outcomes.
Qualifications
- Master's in Pharmacy (MPharm) or equivalent recognised qualification.
- Registration with the General Pharmaceutical Council (GPhC).
- Patient-facing clinical pharmacy services experience.
- Medicines optimisation and medication review experience.
- Ability to work within multidisciplinary teams and communicate effectively.
Responsibilities
- Undertake structured medication reviews and medicines optimisation.
- Provide expert medicines advice to clinicians and patients.
- Support care home and frail patient medication reviews.
- Manage medication queries and complex prescribing issues.
- Contribute to QOF, enhanced services and quality improvement initiatives.
- Support medicines reconciliation after hospital discharge.
