Job summary

The Hull and East Yorkshire Hospitals NHS Trust seeks a Clinical Pharmacist to provide expert medicines support to care homes within the PCN. • Participation in accredited training pathway including independent prescribing.

Responsibilities

• Lead medicines optimisation initiatives in care homes
• Conduct face-to-face and virtual structured medication reviews
• Provide prescribing advice and support to GPs and care teams
• Reconcile medicines post-hospital discharge or admission
• Train care home staff on medication safety and best practices
• Participate in quality improvement projects
• Ensure compliance with CQC, NICE, and NHS standards
• Supervise pharmacy technicians or junior pharmacists where applicable
