Job summary

Qualifications

• Must be a registered pharmacist with GPhC.
• Independent Prescriber status desirable.
• Experience in General Practice or primary care beneficial.
• Strong communication and team-working skills.

Responsibilities

• Deliver expert pharmaceutical care to achieve clinical excellence.
• Lead medicines optimisation and contribute to quality improvement.
• Provide clinical medicines advice to GP practices and staff.
• Support with prescribing queries and medication reconciliation when transferring care.
• Lead on training seminars for Health Care Professionals.
