We are looking to recruit to the post of Clinical Pharmacist to work within our Primary Care Network (PCN) multidisciplinary healthcare team. Southport and Formby PCN comprises of 14 practices throughout the Southport and Formby area. We recognise the value that pharmacists can bring to our practices and our patients, and we look forward to growing our PCN pharmacy team.
The postholder will work as part of a multidisciplinary team, advising and supporting colleagues, in line with local and national guidance, to promote high quality, safe and cost-effective care for patients. They will become part of a group of PCN Clinical Pharmacists and Technicians working under the supervision of the Clinical Pharmacy Team Manager.

Working within GP Practices around the Southport and Formby area and our office in Southport Business Park, the post holder will take responsibility for areas of chronic disease management and undertake clinical medication reviews to proactively manage patients with complex polypharmacy. They will also provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.
The post holder will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare, and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies, so requires motivation and passion to deliver an excellent service within general practice.
Primary care experience would be an advantage, although not essential. Flexibility, enthusiasm, and good clinical knowledge are essential. Successful candidates will be supported in terms of clinical learning and professional development through access to a GP clinical supervisor, other clinical pharmacists and enrolment onto a formal education pathway delivered through the Centre of Postgraduate Pharmacy Education (CPPE) (if not already completed). The post holder will be expected to develop their role to become an Independent Prescriber on completion of the CPPE Primary Care Education Pathway.

-Work as part of a multi-disciplinary team in a patient-facingrole
-Working with GPs and patients to address medicine adherence.
-Review medication and clinical processes for high-riskpatients to ensure high-quality, evidence-based prescribing
-Conduct structured medication reviews (SMRs) to proactivelymanage patients with complex polypharmacy
-Provide clinical medicines expertise to both patients andgeneral practice team
-Improve prescribing practices across the Primary Care Network
-Interface with community and hospital colleagues to helpresolve medication-related issues
-Managing and prescribing for long-term conditions
-Helping the practice deliver on the Local Quality Contract(LQC), Quality, Innovation, Productivity and Prevention (QIPP) and QualityOutcomes Framework (QOF) agenda and enhanced services.
-Being the point of contact for all medicine-related queries.
-Taking over clinical medicines reviews from GPs.
-Audit and education.
-Respect the privacy, dignity, needs and beliefs of patients,carers and colleagues.
-Behave in a manner which is welcoming to and of anyindividual, is non-judgmental and respects their circumstances, feelingspriorities and rights.
