Compensation: £55,000 - £65,000 / year


Description

Passionate about contributing to DMC’s service development across our primary care team and more widely across the organisation, enabling you to be involved in decisions that affect our care delivery. We are looking for enthusiastic and motivated pharmacists to join our team and develop our services. You should be a keen learner, passionate about your clinical development, and able to communicate well at all levels.

The remuneration for this role is £55,000 - £65,000 dependent on experience.


The role

As a clinical pharmacist, your role is crucial in providing expert pharmaceutical care to achieve clinical excellence and improved patient outcomes. In collaboration with healthcare professionals, your primary responsibility is to ensure the safe and effective use of medications.

We are looking for pharmacists who are proactive, self-motivated, and have a positive attitude towards building good relationships.

The post holder will work as part of a multidisciplinary team in a patient-facing role.

Proactively manage patients with co-morbidities.

Support general practice staff with prescription and medication queries, including repeat prescriptions, acute requests, medicines reconciliation, and systems for safer prescribing, while providing expertise in clinical medicines advice.

Provide leadership on medicines optimisation and quality improvement, managing aspects of the quality and outcomes framework and enhanced services.

Participate in clinical triage and manage the GP practice's appointment book.


Skills, Knowledge & Expertise
  • Develop and manage a medicines management plan and deliver patient services as per practice policy and guidance.
  • Manage the medicines management team, including training delivery, to optimise cost-effective prescribing and patient care quality.
  • Attend site visits punctually to meet contractual obligations.
  • Perform audits, including prescription chart screening for appropriateness.
  • Communicate effectively with patients, families, and staff, familiar with site policies and procedures.
  • Undertake medicines management audits to ensure regulatory compliance.
  • Utilise digital tools and software provided by the ICB.
  • Collaborate with healthcare professionals to provide specialist advice, ensuring patient-centred services.
  • Provide clinical training seminars for healthcare professionals.
  • Support policy reviews and changes to drive improvements.
  • Maintain GPhC registration and act according to the code of ethics.
Essential skills:
  • Master’s degree in pharmacy or equivalent.
  • GPhC Registered.
  • Independent Prescriber.
  • Knowledge of GPhC Codes, ethics, and performance standards.
  • Understanding of Care Standards Act and CQC requirements.
  • Ability to work independently, plan, and manage time effectively.
  • High attention to detail and accuracy.
  • Commitment to clinical learning and development.
  • Experience in General Practice.
  • Experience with Emis Web.
  • Basic IT skills in Microsoft Office.
  • Excellent communication skills.
  • Aspiration for clinical excellence.
