Job summary

Health Alliance Primary Care Network in Harrow is seeking a Clinical Pharmacist to join a multidisciplinary team. The role focuses on medicines optimisation, medication reviews, and supporting safe, high-quality prescribing across six GP practices.

The successful candidate will work towards Independent Prescriber status and engage in structured medication reviews, chronic disease management, andPCN priorities within a collaborative primary care environment.

Qualifications

  • Experience as a Clinical Pharmacist within primary care is desirable.
  • Ability to conduct structured medication reviews and support QOF/IIF priorities.
  • Willingness to pursue Independent Prescriber status as part of role development.

Responsibilities

  • Provide structured medication reviews for patients with complex medication needs.
  • Support medicines optimisation and prescribing guidance across the PCN.
  • Deliver patient-facing clinical activities including chronic disease management.
  • Contribute to safer prescribing initiatives and quality improvement work.
  • Review hospital discharge letters and ensure safe medicines reconciliation.
  • Collaborate with GPs, nurses, pharmacy technicians, and care coordinators.

Job description

We have an excellent opportunity for a Clinical Pharmacist to join Health Alliance Primary Care Network, a friendly and supportive PCN made up of six GP practices, caring for a patient population of over 58,000 patients.

We are looking for an enthusiastic and motivated pharmacist who is either already an Independent Prescriber or is keen to develop towards becoming one. This is an exciting role for someone who wants to make a real difference in primary care, working as part of a multidisciplinary team to support safe, effective and high-quality medicines optimisation across our practices.

The successful candidate will play an important role in supporting patients with medication reviews, long-term condition management, prescribing safety, and clinical pharmacy services across the PCN. This is a great opportunity to develop your skills within a collaborative and forward-thinking primary care environment., The successful candidate will work as part of a multidisciplinary team to provide medicines optimisation and prescribing support across Health Alliance PCN. The role will involve patient-facing clinical activity, including structured medication reviews, medication advice, chronic disease management, safer prescribing initiatives, and supporting high-quality prescribing across our GP practices.

The post requires confident autonomous working in a clinical setting, delivering pharmaceutical care to a diverse PCN population. The Clinical Pharmacist will provide medication reviews and advice, with protected time to complete structured medication reviews and support contractual requirements, including QOF, IIF and other PCN priorities., Provide structured medication reviews for patients with complex medication needs, polypharmacy, long-term conditions, frailty or other identified clinical risks.

Support medicines optimisation across the PCN, helping patients to understand and use their medicines safely and effectively.

Provide medication advice to patients, carers, clinicians and wider practice teams.

Support chronic disease management through clinical reviews, medicines adjustment, monitoring and prescribing support, within scope of practice.

Contribute to safer prescribing initiatives, including audits, searches, high-risk drug monitoring, prescribing safety alerts and quality improvement work.

Support achievement of QOF, IIF, CRM and PCN DES and other contractual or local enhanced service requirements.

Work collaboratively with GPs, nurses, pharmacy technicians, care coordinators, social prescribers and other members of the multidisciplinary team.

Review hospital discharge letters, clinic letters and medication changes where appropriate, supporting accurate and safe medicines reconciliation.

Help reduce medicines waste, inappropriate prescribing and unnecessary medication burden for patients.

Support repeat prescribing processes, medicines queries and prescribing governance across PCN practices.

Maintain accurate clinical records and ensure all work is appropriately coded and documented within the clinical system.

Work within professional, legal and ethical boundaries, escalating clinical concerns appropriately.

Engage in continuing professional development and, where not already qualified, work towards becoming an Independent Prescriber if agreed as part of the role development plan.

You must drive, have your own car and live within a 30 minute drive from Harrow.
