A leading medical practice in Croydon seeks an experienced clinical pharmacist to enhance medicines optimisation services. The role involves clinical reviews, managing chronic diseases, and working within a supportive multi-disciplinary team. The ideal candidate will have prior experience in primary care and strong leadership skills, contributing to improving patient care and operational efficiency.
We are seeking to recruit an experienced clinical pharmacist to join our group of PCN pharmacists. We are a 5 practice PCN (Birdhurst Medical Practice, Friends Road Medical Practice, Haling Park Medical Practice, Stovell House Surgery and Violet Lane Medical Practice) with a PCN population of 37,000. Each practice is assigned their own pharmacist; this role is to be based at Violet Lane Medical Practice exclusively.
We are seeking a friendly and reliable clinical pharmacist, preferably with previous experience of working in primary care, to develop and manage medicines optimisation services within Violet Lane Medical Practice. Ideally you will have already completed your CPPE course and preferably your independent prescribers course.

Salary is highly competitive and dependent on experience. Additionally, you will enjoy 6 weeks annual leave per year (pro rata), statutory bank holidays, access to the NHS pension and MDU cover.

The post holder is a pharmacist, who acts within their professional boundaries, supporting and working alongside the clinical team. At Violet Lane we have another part time pharmacist as well as a prescription clerk and are looking for a leader of this team.
The post holder will work as part of a multi-disciplinary team in a patient-facing role. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the GP practice(s).
The post holder will provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.
The post holder will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.
The post holder will be supported to develop their role to become an independent prescriber if not completed already.
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
