A two-year fixed-term clinical pharmacist post with Northolt PCN, based in general practice settings across the primary care network. The work centres on medicines optimisation: medication reviews, long-term condition clinics, medicines advice for clinicians and patients, and reconciling medicines after hospital discharge. It is a patient-facing, non-dispensing role, and fit depends on holding an independent prescribing qualification and GPhC registration.
What the job involves
- Conduct structured medication reviews and long-term condition clinics for patients needing medicines optimisation, and recommend changes to clinicians
- Give medicines advice to GPs, practice staff, other healthcare teams and patients, with follow-up to monitor the effect of changes
- Reconcile medicines on discharge from hospital, intermediate care and into care homes, working with patients and community pharmacists
- Oversee repeat prescribing, repeat dispensing, electronic prescribing and medication review systems, including monitoring tests
- Provide targeted support and proactive review for vulnerable, complex patients at risk of admission or readmission
- Identify cohorts at high risk of medicine-related harm through pre-prepared practice computer searches
- Undertake simple prescribing audits, feed back results and implement changes with the practice team
- Implement changes from MHRA alerts, product withdrawals and local or national guidance, and agree and review formularies and protocols
- Support QOF, incentive schemes, QIPP and other quality and cost-effectiveness initiatives, and CQC compliance where medicines are involved
- Provide education and training to the primary care team and take part in teaching trainee pharmacists and students
- Contribute pharmaceutical advice to new services and analyse medicines data to highlight issues and risks
- Support public health and vaccination campaigns, and refer patients in good time to the right clinician
