Job summary

University Hospital Southampton NHS Foundation Trust invites applications for a Band 6 Clinical Nurse Specialist within the Supportive and Palliative Care Team. You will deliver expert supportive and palliative care to patients with complex needs, working autonomously while collaborating with the multidisciplinary team.

The role supports timely discharge, proactive assessment, and decision-making to improve patient outcomes and experiences.

Qualifications

• Registered nurse on the relevant part of the NMC register.
• Degree qualification (L6) or equivalent (120 credits).
• Post-registration qualification in area of specialty or equivalent experience.
• Completion of all Band 5 (preceptorship) competencies.
• Evidence of supporting/mentoring learners in practice.
• Extensive post-registration experience within the specialist area.
• Knowledge of current issues within the acute health service.
• Ability to communicate clinical information clearly.

Responsibilities

• Provide specialist assessment and personalised care planning for palliative and supportive care patients.
• Coordinate care and ensure effective communication between patients, families and the MDT.
• Support decision-making and advance care planning to align care with patient goals.
• Work with hospital, community and primary care services to facilitate smooth transitions and timely discharge.
• Educate and support health professionals to enhance palliative and end-of-life care.
• Contribute to service development and quality improvement initiatives.
