We are now looking for a Clinical Nurse Specialist to join us on a full-time or part-time permanent basis.

The Role
As a Clinical Nurse Specialist, you will co-ordinate and manage a complex caseload of patients and their families in the community, providing expert palliative and end of life care tailored to individual needs.
Working autonomously or as part of a multi-disciplinary team, you will assess physical, psychological and spiritual needs, plan and implement evidence-based interventions, and offer specialist clinical advice to both patients and professionals.
You’ll communicate complex and sensitive information with compassion, advocate for patient choice, and contribute to anticipatory care planning, referral co-ordination, and risk management.
Additionally, you will:
- Support colleagues through supervision and line management
- Provide informal and formal teaching to health and social care professionals
- Participate in patient triage and referral management
- Help drive the hospice’s clinical governance agenda
About You
To be considered as a Clinical Nurse Specialist, you will need:
- A minimum of five years of post-registration experience
- Experience leading and co-ordinating the care of a patient with complex palliative care needs
- Strong knowledge of palliative care and symptom control
- Excellent communication and clinical decision-making skills
- Current UK professional NMC registration
- A full, valid driving licence and access to a car to travel throughout our defined catchment area (Swindon, Marlborough, Pewsey, Wootton Bassett, Lechlade, Fairford and Shrivenham)
