Job summary

Provide CIC in Chelmsford is recruiting a Band 7 Nurse for the frailty virtual ward. You will hold a Non-Medical Prescriber qualification and an assessment and consultation course, with at least 3 years' community experience, to assess patients at their home and support the team in preventing hospital admissions.

The post is full-time (37.5 hours), across 7 days with shifts 8am-4pm or 12pm-8pm, salary of £49,387-£56,515 per annum, and provides NHS terms.

Job Overview

Here at the frailty virtual ward we are looking to recruit a Band 7 Nurse, who has a non-medical prescribing qualification and assessment and consultation course, to come and join our service. The applicant needs to have had at least 3 years of community experience and an extensive understanding of frailty. The post holder will help to drive the team forward, and support by delivering assessments in the patients normal place of residence. The service works closely with Urgent Community Response Team (UCRT) in helping to prevent unnecessary admissions to hospital and supporting patients to remain at home.
