Mental Health Services Nationally are undergoing significant transformation under the Community Mental Health Framework. Medway Community Mental Health Team forms part of the North Kent Community Services. It is a diverse multi-cultural team that is looking to recruit a band 6 Clinic Nurse to work within the team, providing specialist mental health care for people with multiple and complex needs that require intensive support and to take a lead role in the depot Clinic.
You will be responsible for ensuring the highest standards of physical health care for service users . You are expected to be a good team worker and have the ability to supervise, teach and develop the physical health care skills of the junior members of the team as well as acting as a positive role model for all staff and be able to lead in areas of clinical practice and development
Extensive knowledge of running a clinic a is required, alongside physical health monitoring such as ECG's, Venepuncture, Pochi Machine operation and escalation of physical health concerns to the appropriate staff.
Medications management, administration and physical health checks will provide a high standard of care to service users. There is a strong emphasis on the monitoring and reporting of physical health checks at regular intervals.
As part of the Band 6 Nursing team, you will be required to attain a mentoring qualification.
Main duties of the job
- Carry out and coordinate comprehensive, systematic nursing assessments which take account of relevant mental, physical, social, cultural, psychological, spiritual, and environmental factors, in partnership with service user and others through interaction, observation and measurement.
- You will also be required to write tribunal reports, attendance at CPA meetings, and Tribunal meetings.
- Work collaboratively with other disciplines, service user and their carers to agree a holistic person centred care plan that addresses the needs identified through assessment.
- To deliver and evaluate safe, person-centred care in partnership with the service user andtheir carers to support recovery.
- To act as a clinical leader supporting best practice in line with NICE guidance.
- To facilitate and review approved Quality Improvement initiatives within the care setting
